USA HOCKEY REPORTING SYSTEM

USA Hockey requires a game report for ALL game misconducts, major penalties, and match penalties to be reported through the USA Hockey Reporting System. Please log into your USAH referee profile and complete such game reports there.

MAJOR PENALTIES

All incidents involving a major penalty should be reported immediately following your game or within 48 hours on the USA Hockey reporting system.

GAME MISCONDUCTS

All incidents involving a game misconduct should be reported immediately following your game or within 48 hours on the USA Hockey reporting system.

MATCH PENALTIES

All incidents involving a match penalty must be reported immediately following your game. All officials working the game must submit a report, regardless of whether or not they witnessed the incident.
